GP Management Plan

If you have a chronic medical condition, you may be eligible for a GP Management plan (GPMP). GPMPs helps your GP coordinate your medical care with other health professionals. The GPMP will provide comprehensive, accurate and up-to-date information about your condition and treatment, and you may be entitled to subsidised sessions with allied and other health care providers.
