Chronic Disease Management (GPCCMP)

Our GPs are experienced in managing a wide range of chronic health conditions through the newly introduced General Practice Chronic Condition Management Plan (GPCCPM) items, designed to deliver more coordinated, patient-centred care.

These plans support individuals living with one or more chronic conditions by facilitating structured care, ongoing monitoring, and collaboration with allied health professionals where appropriate.

Conditions we commonly manage include:

  • Asthma
  • Diabetes
  • Hypertension (high blood pressure)
  • Heart disease
  • Osteoarthritis and rheumatoid arthritis
  • Osteoporosis
  • Chronic respiratory conditions (e.g. COPD)
  • Chronic pain

Under the GPCCMP framework, patients may be eligible for Medicare-subsidised access to allied health services to support their treatment goals. Your GP will work with you to develop a tailored plan that aligns with your specific health needs and circumstances.
